PHASE II (Extended Care - Transition) View Schedule
Minimum 15 hours of recovery activities
Phase II integrates employment and/or school activities into the clients treatment plan. In this phase, the client will begin accepting responsibilities in the community and begin making the gradual transition from the program to the community. The client learns to balance daily activities with his/her recovery and is under moderate supervision that allows him/her to maintain employment and/or volunteer services during the day and continue alcohol and drug treatment in the evenings.
The client is introduced to this treatment phase by meeting with administrative staff who will recommend an achievable job-search plan. The client will have access to computers, fax machines, and high-speed Internet. During this transitional treatment phase, clients are required to participate in the job-search program and attend a weekly minimum of two evening Intensive Outpatient Program groups, one private counseling session and continue daily 12-step meetings. The client is responsible for his/her own transportation to and from work/school, groups, counseling sessions, and 12-step meetings.
Treatment staff monitors the clients progress weekly and help with strategies and plans if one is not employed within two weeks. Upon completion of Phase II, the Treatment Evaluation Team will review the clients progress and revise his/her treatment plan for Phase III or prepare a discharge plan.
