Alcohol and Drug Treatment Program Overview
At Able To Change Recovery, we believe that one of the most vital functions of our substance abuse and chemical dependency treatment program is the care planning; it is the individual's road map to recovery. The plan is a practical, understandable tool that the individual, his family, and our team of clinicians utilize together to steer the course in the journey of recovery. It is the focal point of each session with the individual and includes personally defined goals along with realistic objectives that address the issues of one's addiction.

An individual's course of treatment in the program will be based upon the, individualized, person-centered, interdisciplinary care plan. It is reviewed, and as clinically indicated, revised continuously as needed, while the client is in the treatment program. The treatment plan will be developed around goals and objectives. The goals, when achieved, should allow the person to function in their community free from the use of alcohol and drugs. As well, Able to Change Recovery treatment plans also provide support and aftercare to assist the client and his family with maintaining recovery once discharged from the program. All treatment plans provide for the details of an aftercare program.
When ever there are problems which require clinical services not provided by the program, the plan would indicate what referrals and / or other arrangements have been made to address those needs.
The specific short and long term goals and treatment interventions selected will relate to specific discharge criteria established for each person entering the program. There is a high value placed upon client participation in treatment planning and accordingly, the specific plan that is established will reflect a blend of both clinical judgment and client preference.
An Initial Treatment Plan will be established within 3 days of the client's admission to the program. Following completion of all program clinical assessments, the Master Treatment Plan will be completed within 7 days following admission. The next treatment plan review will occur no later than 7 days.
The written plan and subsequent revisions thereof will be established at a treatment-planning meeting. The content of the treatment plan is a clinical responsibility and all disciplines involved in the care of the client are required to provide their input into the treatment planning process.
